An inquiry in the Philippines has blamed human error for the capsize of a ferry in June that left nearly 800 people dead.

The Philippines Board of Marine Inquiry has found the ship's captain sailed toward the eye of a typhoon, miscalculating the risk.

The ferry capsized after hitting a reef off the island of Sibuyan.

The captain was among those killed in the disaster.
